Print how you would like the badge to read and send with a check for $6.00 (add $2.50 to order if you would like to have the badge sent to your home address) to PO Box 455 Addison, IL 60101. Order a badge for your spouse and I can ship two (2) for the $2.50 mailing change.

Delivery may take several weeks since we must accumulate eight badges before submitting an order to the engravers. You will be notified that the badges are ready for pickup at a club meeting or event if you are not having them shipped to you.
